The following forms may be filled out online, saved to your computer, and then emailed to email@example.com. You can also fill out the form, print and sign it, and deliver it to the office and address indicated. Another option is to print the blank form and fill in the fields by hand.
Information Change Form: used to change basic student information such as name, social security number or academic plan
Domicile and Other Colleges Form: used in conjunction with the Information Change Form; this form must be filled out if it has been 12 months or more since the student was last enrolled in MECC
Enrollment Request Form: used to plan student's initial schedule or to add or drop classes during the fall
Transcript Request Form: used to request student's or alumni transcript; this form should be submitted at least 5 business days before the transcript is required.
